Our Practice:
Warner Family Medical Practice has been providing healthcare for the local community in Warner for over 14 years. Our friendly and experienced medical receptionists are there to assist you with your needs, whether it be requesting the transfer of your medical records or booking an appointment with your doctor.

Our practice provides 7 days a week access to your doctor, we believe that having your own doctor is important to your health and where possible, we try to maintain this important interaction.

Our Team:
Here at Warner Family Medical Practice, our team is committed to ensuring that you receive the best care possible, which is why all of our staff members are highly experienced in the medical environment.

Our team of highly experienced nurses work at the direction of our doctors and will assist in providing comprehensive coordinated healthcare services. Our Practice Manager, Simonne, can assist you with any questions that you may have regarding our practice or any of our doctors.  We love to receive feedback on our performance and on how we could do things to better assist meet the needs of our patients.  Please ask our friendly receptionists for a feedback form that can be left in the feedback box near reception.

Practice Hours:  
Here at Warner Family Medical Practice, we have recently extended our hours in an effort to offer a more convenient and timely service to our patients. We want to ensure that our growing community has the health care they require in order to continue going from strength to strength. 

Our new practice hours are:

Monday - Thursday 7am until 6pm
Friday 7am until 6pm
Weekends 9am until 1pm

Our standard appointments last for 10 minutes, if you think you need more time with your doctor please advise reception staff of this. You can learn more about what qualifies as a long appointment by clicking here. We will always ensure that we can find a suitable appointment time with your preferred doctor. Please be aware that emergencies will be given priority, if this situation should occur when you are scheduled to attend the practice, our staff will ensure every effort is made to contact you. 

Due to a high demand of appointments at Warner Family Medical Practice, we do ask that if for any reason you are unable to attend your appointment that you notify us as soon as possible to allow others the opportunity for an appointment with their doctor. In some cases we may charge a fee for cancelled appointments. 

After Hours Arrangements 
When our practice is closed we employ the Australian After Hours Doctors service for all after hour needs, this is a bulk billing service if you are eligible. Please telephone 1300 HOME DR (1300 4663 37). If you have an emergency, please call 000.

Information you supply:
All new patients are asked on arrival to complete a registration form prior to your consultation. To ensure optimal patient care, we request that you inform us of your cultural background. For existing patients, if there have been any changes to your personal information such as change of contact or Medicare details, address or emergency contact please bring this to the attention of our reception staff on your next visit or call our friendly receptionists.

It is the policy of our medical practice, when receiving a significant abnormal test result to contact you to arrange the follow up medical appointment.  We encourage patients to contact the surgery for results of all their investigation.  Your enquiry will lead to the reassurance that result were as expected or, of a course of action that your doctor has indicated.  Results cannot be given over the phone and we advise that a follow up consultation be scheduled with you doctor to discuss any results.

Repeat Prescriptions:
Unfortunately we are unable to action requests for scripts over the telephone and if our patients require a prescription, they are required to consult with their treating doctor. Referrals to specialists cannot be written without a consultation. 

Home visits:
Please speak to our reception team about whether home visits are available to you.

Electronic Communications: 
Here at Warner Family Medical Practice, we want to ensure that you receive the best care possible. It is for this very reason that we ask our patients NOT to email us in the event of an emergency, in the event of an emergency you must call 000We also do not take appointment bookings, requests for scripts or answer any kind of medical queries via email communication. You should always call the practice to make an appointment with a doctor if you would like to discuss your health. Our support staff are not trained to give medical advice to patients or members of the public. 

Your rights:
If you have a problem, we would like to hear about it. Please feel free to talk to your doctor or receptionist. You may prefer to write to us or use our suggestion box. We take your compliments, concerns, suggestions and complaints seriously. However, if you wish to take the matter further and feel that you need to discuss the matter outside of the surgery, please phone the Office of the Health Ombudsman on 133 464 or go to their website to find out more.
